ED Treatment Options
1) Oral Medication Options (PDE-5 Inhibitors)
• Can be taken as needed or as a low daily dose
• Longer duration of action (up to ~36 hours), which may allow for more spontaneity
• Typically taken as needed about 30–60 minutes before activity
• Shorter duration (about 4–6 hours), which some men prefer for situational use
Both medications work by supporting nitric oxide–mediated vasodilation, helping increase blood flow during sexual stimulation. Many men see meaningful improvement when the medication is appropriately selected and dosed.
Common side effects may include headache, flushing, nasal congestion, indigestion, or dizziness. Sildenafil may sometimes cause temporary visual changes (like blue-tinged vision or light sensitivity), and tadalafil may be more associated with muscle aches or lower back discomfort.
Important safety note: These medications should not be used with nitrates, and may not be appropriate with certain alpha-blockers due to the risk of significant drops in blood pressure. We review your health history carefully before prescribing.
2) Injectable ED Options (Intracavernosal Therapy)
- Trimix (alprostadil + papaverine + phentolamine)
- or single-agent alprostadil
What to Know:
- Proper training and individualized dosing are essential
- Common side effects can include mild discomfort, bruising, or tenderness at the injection site
- Less common but serious risks include priapism and tissue changes if injection sites are not rotated properly
Premature Ejaculation (PE)
Premature ejaculation is common and treatable. Many men do well with oral medication strategies. For men experiencing both ED and PE, a tadalafil + low-dose paroxetine blend may be considered.
- Tadalafil supports erectile quality and durability by improving blood flow
- Paroxetine (low dose), an SSRI, can help increase ejaculatory latency time (helping delay ejaculation)
Priapism: A Medical Emergency
