What Is Testosterone Replacement Therapy (TRT)

Testosterone Replacement Therapy (TRT) is a medically supervised treatment designed to restore testosterone to an optimal range when the body isn’t producing enough on its own. Testosterone plays a vital role in energy, muscle maintenance, bone strength, metabolic function, mood regulation, and sexual health—and it’s important for both men and women (in different physiologic ranges).

Many clients notice improvements within weeks, with continued benefits developing over months as levels stabilize.

Ongoing monitoring matters: TRT can have side effects and may require adjustments over time, which is why follow-up visits and lab monitoring are part of responsible care.

Testosterone Options
(And Why Many Plans Use Cypionate)

There are multiple FDA-approved and compounded forms of testosterone used in optimization, and they differ by absorption, duration, and consistency.

Testosterone Cypionate

Testosterone Cypionate is widely used because it can provide stable levels with weekly or twice-weekly dosing, flexible adjustment, cost-effectiveness, and strong clinical track record—especially when paired with lab monitoring and medical supervision. Other delivery methods may include transdermal gels/creams/patches (daily application with variable absorption and transfer risk) or pellets (longer lasting but less adjustable once placed).

Carrier Oils (Seed Oils)

Injectable testosterone is suspended in a carrier oil, which can impact comfort and tolerability. Ascentis providers often prefer grapeseed or MCT oil because they tend to be thinner, better tolerated for subcutaneous or intramuscular injections, and may cause less post-injection discomfort.

Supportive Hormone Tools We May Discuss (When Appropriate)

Because hormone optimization is highly individualized, your plan may include supportive medications depending on labs, symptoms, fertility goals, and tolerance.

Clomid and Enclomiphene are SERMs used in some protocols to support the body’s natural testosterone production by stimulating the hormonal signaling that tells the testes to produce testosterone and support sperm development. They’re sometimes considered for clients who want a non-injectable approach or are concerned about fertility preservation.

HCG may be used in hormone optimization plans to support natural testosterone production and fertility preservation in some men, and it can complement broader endocrine and metabolic goals under supervision.

Anastrozole is an aromatase inhibitor that reduces the conversion of testosterone into estrogen and may be used when estrogen-related symptoms become elevated, guided by lab results and symptoms. Because estrogen plays important roles in overall health, it should be used only with proper monitoring and provider oversight.

Micronized progesterone is a bioidentical form of progesterone used in hormone replacement strategies to restore balance, support sleep and mood, and (when paired with estrogen therapy) help protect the uterine lining for women who have not had a hysterectomy.

Bi-Est combines estradiol (E2) and estriol (E3) and is often used during peri/menopause to address symptoms like hot flashes, night sweats, vaginal dryness, mood instability, and sleep disruption.
